Don't say anything false about supervisor. Also don't give any open feedback to them no matter how much they chase.

I think you don't have to be a super hero for your performance review... if you have done your job on time, got along well with your cross-functional team and with that you managed to achieve something better is what you should be vouching for... in the end we are humans and unless we are not saving lives for our living it's OK to not stress and go with the flow. I will try to make it simple for you. First of all write down what were your set goals for the year and against each goal how you have performed/outperformed. you must have practice of noting down every small achievement throughout the year which will help you out at the performance review meeting.
