Patient Relationship Manager

Role & responsibilities

• Need to be visiting 4-5 patient and staffs for creating a good bond with them
• Coordinate with the current patients of our company and maintain fruitful relationship with them
• Manage and Monitor our Nurses and Caretakers and develop new strategies for arrangement of these resources for the patient
• Process improvement to be done in patient enquiry and patient visit
• Improving the quality of the nurses and caretakers
• Act as the main contact for patients and their families, answering questions and addressing concerns about home care services
• Organize and coordinate home care visits, ensuring that healthcare providers arrive on time and deliver the required services
• Collect the consent form, offer letter with signature from the nurse and feedback from patients and their families about the home care services and use this information to make improvements.
• To make sure If you are Collecting the payment from patient side means you must handover to Accounts team within 24hours
• Quickly address and resolve any problems or complaints that patients or their families might have with the homecare services
• IV and IM having to do patient
• To ensure Mainly focus on cross sales and increasing the revenue
• Manage day works by maintaining a to do list
