Job Description
The Treatment supporters are support the PLHIV seeking care in the private sector to improve data reporting, adherence, retention, viral load testing and support the clinician in their standard practices at the private sector. They will report to the (TC-PSE) based at the state and district level with YRGCARE.
The specific duties will include:
1. Support the PLHIV registration and enrolment into the project
2. Support day-to-day OPD case record and appointment management at the Private clinics for PLHIV and CLHIV
3. Maintain a PLHIV register at the private clinic/hospital and track the PLHIV line list for the refill reminder, LFU tracking and MIS clients and clients due for clinical evaluation or viral load test
4. Provide and support preparedness and regular counselling to PLHIV who are newly registered on ART as we'll as those who are ongoing on treatment under the guidance of the medical provider at the private facility
5. Provide health education and counselling to the patients and family members registered with the engaged provider.
6. Provide counselling of parents and care givers in identifying possible adverse health conditions which may arise from side effects of ART medication or from other infections.
7. Support PLHIV registration, screening of NCD, TB and Malaria
8. Organise counselling sessions for parents/caregivers
9. Conduct follow-up visits / calls to patients for tracking adherence on regular intervals after treatment initiation.
10. Record treatment adherence on a monthly basis and treatment outcome based on the clinical stage and regimen under medical guidance of the private provider
11. Coordinate with the PLHIV and medical provider to ensure index testing of the sexual contacts, children
12. Coordinate with Technical coordinator of ACCELERATE and other government officials and departments to ensure social support and schemes for PLHIV on need based
13. Implementation and management support for partnerships with private partners at district and sub-district levels.
14. Supporting the Technical coordinator to conduct workshops /meetings for improving involvement of private partners in NACP.
15. Establish liaison with public and private providers (dispensaries / hospitals) to promote compliances with national norms, facilitate referral and cross referral to ensure smooth coordination between Govt. ART Centre and private provider in case of PLHIV who are referred from private to public and vice versa.
16. To support the clinician in maintaining the line list of patients in recommended format and to support in monthly/quarterly reporting to JDCSTT of SACS as per the NACO guidelines
17. Ensure that all other data, information and reports demanded from time to time are collected, compiled and delivered promptly to the ACCELERATE supervisor
18. Periodically review the performance of the district and provide regular feedback and support to increase program efficiency.
19. Documentation of best practices of public-private partnership .
Requirements:
- Graduate and above
- Experience in the field of public health.
- Experience in the HIV and AIDS field will be an added advantage.
- Experience in private sector medical practitioners is preferred
- Working knowledge of all computer-based applications (MS Office required)
- Better understanding of English / Hindi & Marathi.
