Roles and Responsibilities
1. IPD Prescription Audit & OPD Prescription Audit (as per NABH Standard)
2. Audit of Drugs in Crash Carts and Narcotic Drugs Stored in Wards
Report in case of discrepancy and conduct corrective & preventive actions.
3. Detection, Investigation, and Reporting of Medication Errors, Including Root Cause Analysis (RCA) and Corrective and Preventive Actions (CAPA)
4. Monitoring the administration of chemotherapeutic medications in hospital wards, ensuring adherence to established protocols and guidelines. In the event of a medication error, promptly identify, assess, and initiate corrective actions to prevent recurrence and ensure patient safety.
5. Detection and Reporting of ADR in the hospital format as well as in the PvPi portal.
6. Monitor compliance
7. To conduct comprehensive training sessions on medication error reporting, as well as the identification, documentation, and management of adverse drug events (ADEs) and adverse drug reactions (ADRs), specifically designed for nursing and pharmacy staff.
8. Provide cross-functional consultation and support to troubleshoot and resolve issues across wards including addressing incidents raised by medical staff related to medications.
9. Non-formulary drugs, when prescribed, were communicated to the attending physician and substituted with formulary-approved alternatives, while ensuring validation of drugs procured through local purchase.
10. Conduct a thorough audit of the restricted Antimicrobial Stewardship Program, collaborating with the relevant physician when necessary to ensure compliance with institutional protocols.
