Medical transcriptionist - Discharge Summary (BLK MAX Hospital)

A. Seek and Email Planned discharges for the day

B. Discharge intimation in concurrence with the Assistant Managers Floor/DMS to Billing Department about the discharge patient and send completed Medical Record File to the Billing Department

C. To make Discharge Summary of the discharges of the assigned department/Ward

D. Handling of the TPA pre auth forms and queries and case summaries for the TPA/CGHS/ECHS/ESI/Panel etc patients

E. Patient file maintenance not limited to:

- Arranging adequate stationary stock related to everyday maintenance.

- Ensuring sequencing.

- Proper binding of sheets.

- Completion of billing papers.

- Tracking of reports/ files/ documents/ certificates/ discharge summary/death summary etc.

- Offloading the nursing staff from doing extra paperwork.

F. Ensuring timely printing of investigation labels & retrieval of reports at all times am/pm shifts, and handing it over to the Team Leader/staff nurse.

G. Placing orders of investigations for printing of labels.

H. Ensure Referral consultation and closure in HIS

I. To assist the doctors in typing the notes in HIS

J. Assist in bed transfer to housekeeping after discharge in HIS

K. Assist in lodging any complaint in the complaint tracker in HIS

L. Inform the dietician about the discharge patients and transfer ins in the ward for dietary advice

M. To maintain Log Book of Discharged patients and transfers of ward

N. To have updated knowledge of their Dept/Ward bed Status (available vacant & occupied beds) and communicate the same to the admission desk (Admission TL)

O. To enter doctor visits, bedside procedures like bedside ECHO ,ECG /any other procedure, equipment used etc in HIS

P. Despatch and receiving the patient files from MRD

Q. Various MISs - as and when required

R. To attach the dietary assessments in HIS to the patient file

S. Any other duties given by the management
