Goal Setting: Collaborating with the patient and their family/caregivers to establish personalized rehabilitation goals based on the assessment findings and the patients priorities. 3. Treatment Planning and Implementation: Designing and implementing individualized treatment plans that focus on improving functional abilities, such as self-care skills (e. g. , dressing, grooming), mobility, coordination, and cognitive skills (e. g. , memory, problem-solving). 4. Adaptive Techniques and Equipment: Teaching and training patients in the use of adaptive techniques, assistive devices, and adaptive equipment to enhance independence and safety in daily activities. 5. Cognitive Rehabilitation: Providing interventions to address cognitive impairments, such as memory deficits, attention deficits, executive functioning challenges, and perceptual deficits, using evidence-based cognitive rehabilitation techniques. 6. Sensory Integration: Addressing sensory processing difficulties through sensory integration techniques to improve sensory modulation, discrimination, and adaptive responses to sensory stimuli. 7. Psychosocial Support: Providing emotional support and counseling to patients and their families/caregivers to cope with the emotional and psychological challenges associated with neurological conditions. 8. Education and Training: Educating patients, families, and caregivers about neurological conditions, rehabilitation strategies, and techniques to facilitate ongoing progress and independence post-discharge. 9. Collaboration and Multidisciplinary Teamwork: Working closely with other members of the rehabilitation team, including physicians, physical therapists, speech-language pathologists, psychologists, and social workers, to coordinate comprehensive care and achieve integrated treatment goals. 10. Outcome Evaluation: Continuously assessing and documenting the patients progress towards rehabilitation goals, adjusting treatment plans as needed to optimize outcomes and facilitate transition to community-based settings
