Assessment : Conduct comprehensive assessments to understand the client's needs, situation, and goals. This may include physical health, mental health, social, and environmental factors. Planning : Develop individualized care or service plans in collaboration with the client and other stakeholders, such as healthcare providers, social workers, or legal professionals. Resource Coordination : Connect clients to the necessary services, resources, and support systems, including medical care, housing, counseling, and legal assistance. Advocacy : Advocate for the client's rights and needs, ensuring that they have access to quality care and services. Monitoring : Continuously assess the progress of the client's plan and make adjustments as needed. Ensure that services are being delivered effectively. Documentation : Maintain detailed records of client assessments, plans, services provided, and progress, ensuring compliance with legal and regulatory requirements. Crisis Intervention : Provide immediate assistance and support during crisis situations, such as mental health crises or legal issues.
