Resource Coordination : Connect clients to the necessary services, resources, and support systems, including medical care, housing, counseling, and legal assistance. Advocacy : Advocate for the client's rights and needs, ensuring that they have access to quality care and services. Monitoring : Continuously assess the progress of the client's plan and make adjustments as needed. Ensure that services are being delivered effectively. Documentation : Maintain detailed records of client assessments, plans, services provided, and progress, ensuring compliance with legal and regulatory requirements. Crisis Intervention : Provide immediate assistance and support during crisis situations, such as mental health crises or legal issues.
