Case Manager

PrivateCourt is looking for Case Manager to join our dynamic team and embark on a rewarding career journey Assessment: Conduct comprehensive assessments to understand the client's needs, situation, and goals

This may include physical health, mental health, social, and environmental factors

Planning: Develop individualized care or service plans in collaboration with the client and other stakeholders, such as healthcare providers, social workers, or legal professionals

Resource Coordination: Connect clients to the necessary services, resources, and support systems, including medical care, housing, counseling, and legal assistance

Advocacy: Advocate for the client's rights and needs, ensuring that they have access to quality care and services

Monitoring: Continuously assess the progress of the client's plan and make adjustments as needed

Ensure that services are being delivered effectively

Documentation: Maintain detailed records of client assessments, plans, services provided, and progress, ensuring compliance with legal and regulatory requirements

Crisis Intervention: Provide immediate assistance and support during crisis situations, such as mental health crises or legal issues
