Lack of coherence in global leadership. The same org structure which helps getting exposure bottoms up, prevents collaboration from top down as people with same job responsibilities and designations end up reporting to different set of top management who in turn have different agendas and priorities. Finally lack of an overarching documented business system makes tasks feel more tactical, adhoc and reactive, rather than strategic and proactive.
