Job Description: Heart Failure Counselor (Dietician)

Job Summary:

We are seeking a dedicated and compassionate Heart Failure Counselor (Dietician) to join our team. In this role, you will work closely with cardiologists in the outpatient department (OPD) to provide personalized counseling to heart failure patients. Your primary responsibility will be to offer dietary guidance, track patient prescriptions, and input relevant data into the clinician app. You will play a key role in supporting patients health management by ensuring that they understand their treatment plans and dietary needs.

Key Responsibilities:

Counsel patients in cardiologists' OPD on nutrition, lifestyle changes, and disease management.

Track and monitor prescriptions provided by cardiologists, ensuring accurate recording in the clinician app.

Input and update patient data in the clinician app for tracking prescriptions and progress.

Provide education to patients about their treatment plans, medication adherence, and dietary adjustments to improve heart health.

Shift Information:

* This position requires flexibility, as you will be expected to work in two shifts (morning and evening), depending on the OPD schedule.

Qualifications:

* Bachelor's degree in Dietetics, Nutrition, or a related field (Registered Dietician preferred).

* Proven experience in counseling patients, ideally in a clinical or healthcare setting.
