Position Overview:

The role will oversee the activities of Health Coaches, Care Managers, and Mental Wellness experts. The role involves educating and training the team to provide the best possible care for patients with Acute Coronary Syndrome (ACS) and Heart Failure (HF). The ideal candidate should have an MBBS-MD degree having a significant experience in cardiac rehabilitation, demonstrating clinical expertise and a passion for patient care.

Responsibilities:

1. Clinical Leadership Oversee and manage clinical operations of the cardiac rehabilitation program. Develop and implement clinical protocols and guidelines in line with best practices. Ensure high-quality patient care and adherence to clinical standards.

2. Training & Upskilling Lead, mentor, and train a multidisciplinary team of health professionals including Dieticians, Care Managers, and Mental Wellness experts. Conduct regular performance reviews and provide continuous professional development opportunities. Foster a collaborative and dynamic team environment.

3. Patient Care & Program Management Supervise patient assessments, care plans, and rehabilitation progress. Monitor patient outcomes and implement strategies to enhance efficacy. Drive patient engagement and satisfaction to increase referrals and renewals.

4. Clinical Innovation Leverage data and technology to enhance patient care and operational efficiency. Collaborate with technology teams to develop new tools and solutions for cardiac rehabilitation. Stay updated with the latest clinical research and incorporate relevant findings into practice.

5. Digitization/Summarization of Records received from Patient Discharge/prescription.

6. Oversee and manage clinical operations of the cardiac rehabilitation program.

7. Training of Ops team to make conversations with patients more clinical.

8. Supervise patient assessments, care plans, and rehabilitation progress.

9. Monitor patient outcomes and implement strategies to enhance efficacy.

10. Drive patient engagement and satisfaction to increase referrals and renewals.
