Nursing Team Leader

To take ICU rounds ,Assess clinical condition and identify patients plan for plan of discharge . Clinical Assessment of patients requiring homecare services -CCN ,Palliative , EOLC and Procedures . Case discussions with doctors and clinical team members for plan of care /goals Meeting doctors regularly for coordinating patient care and conversion of leads . Clinical Assessment in hospital and out of hospitals with patients diagnosis ,lines and tubings ,Vitals , clinical condition in short . Documentation -Assessment form ,Package and end to end coordination . Once Service is confirmed documents to be duly filled and signed ,registration ,KYC ,Pt agreements and other consents as required . Coordinating with other clinical team ( TL/Supervisor ) for home assessment and and Onboarding patient at home . Addressing esculations received to the concerned person Maintaining relevent trackers Regularly updating doctors on ongoing patients Being a part of all all training ,Meeting and calls Information of service discontinuation to be mentioned in relevent group Coordinating with nobin for CCN Packages and rewards .
