Role & responsibilities

• Prescription Audit
• Detection of Medication Errors
• Drug Interaction Screening
• Adverse Drug Reaction monitoring and reporting
• Medication Reconciliation audit
• AMS: optimizing and monitoring antibiotic prescribing, infection prevention, education and training
• Training for Clinical Staff
• Assessment of drug related problems
• Screening new patient medication order for potential problems such as drug allergies, drug interactions, Diluent incompatibilities and informing the physicians about the alternatives.

Preferred candidate profile

• Should have knowledge of NABH SOPs and Policies and Quality Indicators
• Providing Drug Information as needed
• Storage Audit: auditing of ward stocks and Crash cart
• Promotes the selection of rational drug therapy and support hospital formulary development process by consulting with Doctors and Nurses
• Therapeutic substitution of drugs and their brands as needed
• Responsible for overall medication management according to NABH standards
• Attending the ward rounds with Physicians: Providing Drug Information, Monitoring the Drug Therapy, monitoring the patients development, Providing the recommendations, saving costs and improving the quality of health care services
• Assist in achieving key results as defined by the business plan of the organization.
• Willing to take up new responsibilities as and when required.
