Defects found in production should be immediately reported and a plan should be made to fix the issue.

Report the defect to the development team and stakeholders
Analyze the impact of the defect on the system and users
Prioritize the defect based on severity and impact
Create a plan to fix the defect and test the fix thoroughly
Deploy the fix to production after testing
Monitor the system to ensure the defect has been resolved

If any bug found in production , then verify that
1. For that particular , module , scenario , do we have any any cases , have you covered that part in testing , Was the bug in scope.

Take the steps how the produce that bug.
Check in RTM did QA team tested it , what was the result , Have they locked anything regarding same in jira
If its missed from QA team then they have to check and produce that on QA env.
Inform dev team ,and take quick actions to solve that Bug based on priority .and QA team should test on priority .and QA team should also check the other scenarios regarding same ,also impact on other modules or page or Functionality before deploying again to production
If the bug was not in scope then inform Manager PO regarding same and discuss to take the issue in next sprint as per priority set by them.

While testing if you observed any bug , add this bug to the defect tracking tool imeeduately
