Senior Manager (Operations) - Home Care

Role & responsibilities

• Lead and manage the home care department, ensuring that all operations align with the hospitals mission and standards.
• Supervise and guide home care staff, including nurses, caregivers, and other healthcare professionals.
• Ensure proper training and development of staff members to maintain a high level of competency in home care services.
• Oversee the planning, coordination, and implementation of home care services for patients after discharge or for long-term home-based care.
• Monitor patient progress and ensure that care plans are followed and adjusted as necessary.
• Act as a liaison between patients, families, and medical staff to address patient needs and concerns.
• Develop and implement policies and procedures related to home care services in alignment with hospital protocols and healthcare regulations.
• Maintain quality assurance and performance improvement initiatives for home care programs.
• Manage home care resources, including equipment, supplies, and personnel, ensuring they are available when needed.
• Oversee the budget for the home care program, ensuring cost-effective operations without compromising care quality.
• Coordinate with hospital departments, such as discharge planning, physical therapy, and social work, to ensure smooth transitions for patients from hospital to home care.
• Communicate effectively with patients and their families regarding the care process, expectations, and follow-up.
• Address patient concerns promptly and work toward resolving issues to maintain high satisfaction levels.
• Conduct regular assessments and audits to ensure the quality and effectiveness of home care services.
• Implement feedback mechanisms to continually improve the patient experience and outcomes.
• Stay updated with the latest developments in home healthcare practices and integrate them into service offerings.
• Provide ongoing education and training to home care staff on best practices, safety protocols, and new healthcare trends.
• Educate patients and families about care needs, medications, and health management in the home setting.
• Monitor billing and reimbursement processes for home care services, ensuring that services are documented accurately for insurance and regulatory purposes.
• Ensure financial sustainability of the home care program by managing costs and exploring opportunities for growth.
• Build and maintain relationships with external healthcare providers, insurance companies, and community organizations to ensure comprehensive care delivery.
• Collaborate with physicians and specialists to create appropriate and personalized care plans for home-based patients.

Preferred candidate profile

• Bachelor's degree in Healthcare Administration, or a related field.
• 12-15 years of experience in healthcare, with at least 4-5 years in home care management or a similar leadership role in a healthcare setting.
• Experience in managing or supervising a home care program or department, ideally within a hospital or multidisciplinary healthcare environment.
• Experience in managing teams of healthcare professionals, such as nurses, therapists, and caregivers.
• Strong leadership and management abilities to effectively lead a diverse team and manage home care operations.
• Deep understanding of patient care protocols, discharge planning, chronic disease management, and home-based healthcare services.
• Exceptional communication skills for interacting with patients, families, medical staff, and external stakeholders.
• Strong commitment to providing high-quality, compassionate care to patients in the home setting.
• Ability to identify challenges within home care delivery and find effective solutions, especially in remote or non-traditional care settings.
• Experience in budgeting and cost management for home care services.
