1. To treat every patient with courtesy and exceptional efficiency and compassion.
2. Facilitate the care team in coordinating care for visits and for future healthcare needs and ensure that proper care has been provided to each patient as per plan.
3. Handle calls from patients if needed. Resolve the reason for the call or route to the appropriate party.
4. Provide an effective communication link between patient and medical staff, including relaying messages from providers, gathering information from patients for providers, etc.
5. Support in medication refill process. Ensure that information goes when and where it is needed.
6. Coordination with clinical and non clinical support services (Counseling, OT, Admission, Billing, House Keeping, Pharmacy, and Maintenance) to facilitate the IP Care.
7. Ensure that all patients are tracked and data entered into systems for follow-up and reporting.
8. Act as a back-up to other Patient Care Coordinator or to other Care Team members as needed.
9. Facilitate the OPD process as per policy.
10. Act proactively in quality improvement program.
11. Proper PSI (Patient Satisfaction Index) taken from each and every patient
12. Handling every patient from entry to exit.
13. Floor management
14. Prompt attention to the patient queries and assist them immediately according to their needs.

18. Informing the patients about the OPD Protocols & the facilities available.
19. Educating the patients about their rights & responsibilities.
20. Answering the queries of the patients
21. Guiding or directing the patients to the waiting area or department or Doctor as per the signages.
22. Should have the knowledge of consultant roster- OPD Days/OT days/Timings etc
23. Categorizing the patients as per their disabilities, informing all the hospital staff & availing all the basic facilities (wheelchair assistance) & priority in treatment.
24. To respond to all the emergencies as per the code trainings & to help in documenting the incident.
25. To ensure the proper documentation of MLC & LAMA cases.
26. Respect the privacy & maintain the confidentiality of the patients records.
27. Following the instructions as per SOPs & Hospital policy
28. To attend the training programmes organized by the management from time to time.
29. Taking care of patients and checking status and passing information to the attendants.
30. Assisting patients in getting admission and giving appropriate guidelines and information according to the hospitals policy and protocol.
31. Perform other duties as assigned by relevant authorities.,
