Team Lead will be responsible for the coordination of nursing services during assigned shifts by providing administrative and clinical direction or redirection of nursing personnel and coordinating immediate responses to emergency situations (administrative and clinical judgments). Ensuring workforce compliance to policies and protocols and maintenance of high quality Nursing care standards. Team Lead will act as a communication liaison between top Nursing Management and the Nursing Staff.
1 Administrative & Professional:
Organizes and co-ordinates nursing care activities of the department according to the hospital policies and service needs.
Coordinate staff assignments to insure that sufficient but not excessive staff is available to meet the needs of the patients in an effective manner.
Ensure that the unit nursing staff receives sufficient in-service education, training
and development to meet their needs for self development.
Supervise nursing activities verifying that medications are given as ordered and
appropriate documentation is maintained to meet physician needs and regulatory requirements.
Orientate the new staff to the unit and their adaptation to the new environment
and monitor their progress.
Act as a mentor for the new staff and demonstrates the procedures to the new staff and evaluate their competency by return demonstration.
Maintaining service records of nurses in her unit.
Keep the Nurse Manager or Senior Nurse Manager informed of the needs and any problems in the unit.
7.2 To review and ensure that:
Recognizes or anticipates problems on the shift and re-allocates staff between
units or changes staff assignments as needed in response to changing patient
conditions, emergency circumstances, or staff illness.
A well planned duty roster is available at all times.
The punctuality of nursing staff is monitored on a daily basis.
Nurses are in proper uniform.
Round of the hospital is taken regularly and whenever required.
Availability of life saving drugs in all units
Availability and functioning of life saving equipments.
Ensure that no eatables are allowed /used at nursing stations or in the Unit
refrigerators.
7.3 Patient Welfare & safety:
Meet with the top nursing management to achieve nursing care goals.
Participates in the periodic evaluation and reviews the policies and procedures manual.
Keeps staff informed of changes in policies and procedures.
Take departmental round and visit patients and interact with patient and families regularly.
Listen to patients complaints and resolve the same immediately or escalate the same to Nurse Manager or Senior Nurse Manager if further intervention is necessary.
Responds to all Emergency situations, including Code Blue and Trauma calls and keeps an updated report.
Receives and reviews reports on seriously ill patients, new admissions, deaths and any unusual situation and is readily available to assist.
Aware of safety issues, taking action or reporting issues to appropriate personnel.
Assist to review policies and procedures regarding the adequate protection of patients rights.
7.4 Quality of Care:
Help the nursing administration team to review the quality and appropriateness of nursing care and other health-related services and to provide appropriate feedback.
Participate in the units quality improvement process.
Ensure infection control practices and specific infection control policies to be incorporated into unit policies and procedures.
Check if standards of care are maintained and patients are nursed in a clean, orderly and safe environment.
Gives counselling and guidance to nursing staff.
Maintain a discipline among Nurses and support staff.
Help the facility provide a safe and caring environment.
Ensures optimal utilization of resources.
Participates in the monitoring of care within the unit through a continuous quality improvement/assurance program that encourages self-evaluation.
Maintains a good working knowledge of JCI, NABH, state and national standards for hospital care and ensures that the unit meet the acceptable standards of care.
7.5 Patient Charts:
Review and Monitor proper documentation in various charts.
Ensure that all medications are charted correctly.
Any reaction noted of Blood Transfusion/ IV fluids/Medicines and their reporting to nursing office and completion of incident report.
7.6 Monitor and ensure that proper records/registers are maintained in the unit:
Staff Assignment Register
Register to record all educational activities for unit staff
Report Book
Daily Patient census book /Mid night census register for the unit
Indent register for Laundry, pharmacy, medical and general store
Narcotic File and Register
7.7 Education, Information & Communication:
Promotes his/her professional development through self directed and continuing education.
Communicates staff concerns to top nursing management & vice versa in a proper manner.
Promote a learning culture within the unit by educating, informing, and communicating.
Provide information to help the unit provide care consistent with current standards of practice.
Decides and recommends personnel and material requirement for running various Nursing functions.
Evaluates confidential staff reports and makes recommendations for promotion.
Helps in planning departmental staff development programme and arranges for in-service education and orientation programme etc.
Help the facility develop medical information and communication systems with staff, patients, and families and others.
Maintain knowledge of the changing social, regulatory, political, and economic factors that affect medical and health services of long term care patients.
Ensure that the department functions as comforter, provider of direct care and promotes comfort of client.
Involvement in activities that involve knowledge and sensitivity to what matters and what is important to clients.
Shows concern for client welfare and acceptance of the client as a person irrespective of class, creed or gender.
Informs medical staff about changes in policies and procedures and keeps them abreast of changes in JCI, NABH, state, national and international regulations in the unit.
Assess the continuing education needs of personnel and assist them to obtain certifications in the respective programs.
Assures time and access to in-service training for staff.
Encourages special certification of staff members when appropriate.
7.8 Employee Health:
Fosters a sense of self growth and professionalism among employees.
Participates in activities of the safety committee.
7.9 Community:
Helps articulate the long term care facility s mission to the community.
Acts as an advocate for the facility and assists the community in achieving a realistic understanding of the facility s capabilities and services.
Represents the facility in the event of untoward event.

Ensures that ethical and legal rights, privacy and confidentiality of patients are respected.
