Bachelor s degree in Nursing, Healthcare Administration, or a related field (preferred)
EXPERIENCE:
Prior experience in patient care coordination or healthcare management is an added advantage
JOB RESPONSIBILITIES:
Act as the main liaison between patients, their caregivers, and consultants.
Ensure that patients have all required information and documentation for admissions.
Guide patients through the registration and billing processes to minimize wait times.
Coordinate diagnostic tests and ensure timely reporting to consultants.
Facilitate EMD admissions while ensuring compliance with protocols for smooth processing.
Conduct follow-up calls post-discharge to verify patient well-being and coordinate any required follow-up visits.
Facilitate the discharge process directly in the patient s room or ward without the need for patients to visit counters.
Maintain comprehensive documentation and records relevant to patient interactions and care coordination.
Collaborate with various departments, including the Admission Counselling Cell, Mediclaim, Billing, Nursing, and Diagnostics to ensure cohesive patient management.
Uphold patient confidentiality and ensure thorough communication with relevant parties throughout the patient journey.
Report to the Deputy Medical Administrator on all Care Manager-related activities while coordinating with the Assistant Medical Administrator as necessary.
Any other work/responsibility assigned by the HOD/Management in the course of business/work whether within the same unit/dept or any other unit/dept from time to time.
