Monitor and document changes in patients' conditions and promptly report any concerns to the appropriate healthcare professionals.Care Planning: Collaborate with the healthcare team to develop individualized care plans for patients, considering their medical conditions, preferences, and resources. Ensure that care plans are regularly reviewed, updated, and implemented effectively.Medication Administration: Administer medications following established protocols and guidelines. Ensure accurate dosage, proper administration techniques, and appropriate documentation of medication administration.Patient Support and Education: Provide emotional support and educate patients and their families about their conditions, treatments, and self-care strategies. Answer their questions and address their concerns, promoting a positive patient experience.Wound Care: Perform wound assessments, dressings, and other related procedures as required. Monitor wound healing progress and collaborate with the healthcare team to implement appropriate interventions.
