Exclude- there are two types exclude 1 and exclude 2

If physician mention two diseases in a report among one is mentioned as exclude 1 - in that case only one code is taken we shouldn't take exclude 1 code.

If physician mention two diseases in report among one is mentioned as exclude 2 then we should take both the disease and exclude 2.

Combo codes are the codes which is a combination of more than one code, example diabetic retinopathy.

Sepsis icd guidelines

If sepsis is mentioned in the report but organism is not mentioned in that case we use unspecified code for microorganism. Sepsis codes series range from A41 to A49.

Organ failure due to sepsis - severe sepsis

Organ failure and sepsis - both organ failure and sepsis codes

Sepsis due to multiple organisms, mention all the organisms codes which cause the sepsis.
