The Referrals position plays a crucial role in our organization by managing and coordinating all aspects of the referral process to ensure seamless and efficient access to services for our clients. This role is essential in facilitating the smooth transition of clients from one service area to another, ultimately contributing to the overall success of our organization.
Key responsibilities
Review and process incoming referrals from various sources
Communicate with clients and external agencies to gather necessary information
Ensure all referral documentation is complete and accurate
Coordinate appointments and services for referred clients
Track and monitor referral status and outcomes
Collaborate with internal teams to ensure the timely provision of services
Provide support and guidance to clients throughout the referral process
Maintain accurate and up-to-date records of all referrals and related activities
Generate reports and statistics related to referral activities
Participate in continuous improvement efforts to enhance the referral process

Required qualifications
Bachelors degree in a related field or equivalent work experience
