Job Summary:

The Rehabilitation Nurse plays a crucial role in the multidisciplinary team, working closely with patients, families, and healthcare providers to develop and implement personalized care plans that promote recovery, independence, and optimal quality of life. This role involves assessing patients' needs, administering treatments, and providing education to help patients adapt to their physical limitations and improve their overall health and well-being.

Key Responsibilities:

1. Direct Patient Care:
   Administer medications, wound care, and other prescribed treatments.
• Assist patients with activities of daily such as dressing, and mobility.
• Monitor patients' progress and adjust care plans as necessary.
• Provide support and guidance to patients in adapting to new physical limitations.
2. Education and Support:
   Educate patients and their families on recovery processes, treatment plans, and the use of adaptive devices.
• Offer emotional support and counselling to patients and families to help them cope with the challenges of rehabilitation.
• Advocate for patients' needs and preferences in care planning and treatment decisions.
3. Documentation and Reporting:
   
• Maintain accurate and detailed patient records, including assessments, care plans, and progress notes.
• Communicate effectively with the healthcare team, providing regular updates on patient status.
• Ensure compliance with all relevant laws, regulations, and standards of care.
4. Professional Development:
   
• Stay updated on best practices in rehabilitation nursing and continue professional development through ongoing education.
• Participate in quality improvement initiatives and contribute to the development of policies and procedures.

Working Conditions:

• Environment: Work primarily in a rehabilitation facility, hospital, or outpatient clinic setting.
• Hours: May require shift work, including nights, weekends, and holidays.
• Physical Demands: Frequent walking, standing, and lifting as part of patient care duties.
