We are Hiring For International Call Support - Outbound

Job Summary:

We are looking for dedicated professionals to join our International Voice Process - Transitional Care Management team. The role involves making outbound calls to recently discharged patients to ensure continuity of care, provide post-discharge guidance, and support care coordination. It's a structured approach designed to ensure patients experience a smooth and well-coordinated transition between different care settings, such as from a hospital to home. The goal is to reduce complications, avoid readmissions, and provide continuous care during the transition period. The ideal candidate should possess strong communication skills, empathy, and a passion for patient care.

Key components of TCM include:

• Communication: Contacting the patient or caregiver within two business days after discharge.
• Follow-up Visit: Conducting a face-to-face visit within 7 to 14 days, depending on the complexity of the patient's condition.
• Care Coordination: Managing and coordinating services, including medication reconciliation, scheduling follow-up visits, and educating the patient and caregivers.

By ensuring continuity of care, TCM helps improve patient outcomes, reduce healthcare costs, and enhance the overall quality of care.

Key Responsibilities:

• Make outbound calls to patient’s post-hospital discharge to check on their recovery progress.
• Provide guidance on medication adherence, follow-up appointments, and symptom management.
• Identify any health concerns or complications and escalate them to healthcare providers as needed.
• Educate patients about available healthcare services and transitional care programs.
• Maintain detailed records of patient interactions in the CRM or electronic health record system.
• Adhere to healthcare compliance standards, including HIPAA and data confidentiality regulations.
• Collaborate with healthcare professionals to ensure proper patient follow-up.

Required Skills & Qualifications:

• Education: Minimum High School Diploma / Graduate (Healthcare-related background is a plus).
• Experience: 1-2 years in an international BPO, healthcare process, or outbound calling role (Fresher’s with strong communication skills).
• Strong verbal communication skills in English (US/UK accent) with active listening and empathy.
• Ability to handle sensitive patient information with professionalism and confidentiality.
• Comfortable working in night shifts and handling international patients.
• Basic computer skills.
